MEASURING SAFETY PERFORMANCE

We have built a reputation for outstanding safety performance, and release quarterly measurements of our safety performance.

LIFE SAVING

In 2022, we codified a set of common HSE practices, and how to reduce risk, in what we call our Life Saving Rules. These rules were developed based on a two-year review of our high-hazard tasks and associated incidents. We reviewed incidents that resulted in a range of consequences, from days away from work (DAFW) to no consequence. We not only took a deep dive into incidents that had the potential for fatality or DAFW, but we also looked closely at incidents that required medical attention or first aid, near-hit cases and unsafe behaviours.

AREAS OF FOCUS

As safety is the foundation of all Odira energy Karang Agung operations, our Areas of Focus are the foundation of our approach to safety.

These processes are implemented by our operating groups and aim to prevent injuries and damage to equipment and the environment, thereby improving our safety culture and overall health, safety and environmental (HSE) performance.

LEADERSHIP

Key Principles

  1. Actions of senior management develop and drive our safety culture.
  2. All injuries and environmental incidents are preventable.
  3. Line management owns HSE performance—engagement is critical to our success

Major Elements

  1. Operations leadership, senior vice president, vice president, general manager and local management teams developed
  2. Established targets and objectives—decision making and control of necessary resources
  3. Evaluation and coordination of HSE activities
  4. Development and implementation of preventative measures
  5. Reviews of current performance measures
  6. Guidance for operational support
  7. Focal point for employee and/or operational feedback
  8. Management site visits

FATALITY PREVENTION

Key Principles

  1. Identify, assess and manage the risks with the highest potential to result in a fatality.
  2. Focus on minor incidents and critical behaviours with the potential to result in a fatality.
  3. Focus attention and resources on areas determined to be of greatest risk.

Major Elements

  1. High Hazard Tasks
  2. Life Saving Rules and associated processes
  3. Fatality Prevention Behaviours
  4. Consequence management
  5. High-potential incident reporting and analysis

INCIDENT MANAGEMENT

Key Principles

  1. Create an environment where the identification and reporting of at-risk behaviours and unsafe conditions is encouraged.
  2. Utilize root cause analysis tools on incidents identified to involve the greatest potential and highest risk.
  3. Develop and implement corrective actions focused on engineering controls and/or systemic processes.

Major Elements

  1. Incident investigation
  2. Root Cause Analysis (RCA)
  3. Evaluation and coordination of HSE activities
  4. Self-verification

EMPLOYEE ENGAGEMENT

Key Principles

  1. Create a workplace where employees recognize and address at-risk behavior, unsafe conditions, and hazards.
  2. Promote an environment where all employees feel free to speak up and point out potential hazards without fear of retribution.

Major Elements

  1. Improved communication
  2. Setting expectations and monitoring performance
  3. Encouraging the continuation of safe behaviours
  4. Local management teams
  5. Behavioural observation process
  6. Employee peer-to-peer observations
  7. Near-hit reporting
  8. Hazard identification
  9. Safety meetings/toolbox talks

TRAINING AND COMPETENCY

Key Principles

  1. Provide our executives, leaders, and operators with the knowledge, skills and abilities to integrate HSE into the fabric of our operations.
  2. Establish training and related competency processes based upon performance standards and expectations

Major Elements

  1. HSE supervisor training
  2. Managing for world-class safety
  3. New employee orientation
  4. HSE competency and Personnel Certifications
